FP 2010 Reference Form Amended

The online reference form for the Foundation Programme 2010 recruitment round has been slightly amended in response to feedback from stakeholders, and in consultation with NHS Employers.
The nine reference domains now include only two options: “Cause for Concern” or “Acceptable”.
If a referee indicates that there is a cause for concern in one of the domains, they must provide examples of the applicant’s behaviour to support this rating. Updated guidance is provided on the form.
On average, each reference form will take referees approximately three to five minutes to complete. Click here to view a sample form.
Please note that referees will only be able to submit references online. Paper copies will not be accepted.
